How to Enter

All applicants must submit the following by the March 1st entry deadline:

1. Entry Video (**only YouTube videos and DVDs will be accepted this year**)
2. Registration Form
3. Entry Fee: $10 per participant (no charge for accompanists) with -

$20 maximum per individual (with multiple entries)

$50 maximum per groups

Ways to submit your Entry Video: (there are only two ways to submit video entries this year)

E-mail cchastalent@gmail.com a link to a YouTube site where you have uploaded your video.

Mail a DVD (preferably in Windows Media Player or QuickTime format) to:

Ways to submit Registration Form:

Open document by clicking on either Microsoft Word 2003 or Microsoft Word 2007
Click "Download" at the top of the page
Complete form and save the document (use1/2 inch margins if lines do not format correctly)
E-mail saved document as an attachment to cchastalent@gmail.com

Open document by clicking on either Microsoft Word 2003 or Microsoft Word 2007
Click "Print" to print off a hard copy (use1/2 inch margins if lines do not format correctly)
Complete form and mail it to:

Ways to submit Entry Fee: ($10 per participant with $50 maximum (no charge for accompanists))
